Gas Appliance Issue Notification form

Type A appliance technicians can use this form to notify ESV of trends in gas appliances or components failures they are aware of.

Depending on the nature of the notification ESV may actively investigate the matter or monitor the situation to identify and understand trends in the data.

Gasfitter hotline

This form does not replace the 24/7 Emergency hotline for gasfitters to notify ESV of dangerous gas installations. The hotline number is 1800 652 563, then select option 5.
